We are looking for dedicated professionals to be a part of our Skilled Nursing department. As an LVN, you will serve as a charge nurse in the assigned Health Care Center (HCC), under the supervision of the Director of Nursing. You will assist the supervisor in planning, organizing, developing, and directing day-to-day HCC operations, participate in resident needs assessments, develop and coordinate resident care plans, and perform required nursing procedures associated with the designation as a Licensed Vocational Nurse (LVN).
WhatYou Will Do
- Supervise and provide nursing care in accordance with physician orders and recognized nursing techniques and procedures, established standards, and administrative policies.
- Perform duties related to pre‑admission, admission, and discharge of residents.
- Manage medication regime, including acceptance of physician’s orders, transcription, ordering, and ensuring medications are on hand.
- Ensure medical supplies are accessible and identify need for additional supplies in a timely manner.
- Maintain a safe environment for residents.
- Monitor and supervise Health Support Assistants (HSA), ensuring appropriate care and meeting resident needs.
- Participate in various Quality Assurance Performance Improvement Committees and assigned tasks.
- Communicate with family members, providing information about resident care and condition, and coordinate with physicians, other staff, support services, and outside agencies.
- Record all pertinent information, including observations, treatments, and medications on the resident Electronic Medical Record (EMR), and keep the supervisor informed of any adverse changes in resident condition.
- Perform other duties as assigned by the supervisor.
Required:
- High school diploma or equivalent.
- Must be an LVN in the State of Texas and in good standing with the Board of Nurse Examiners.
- Possess current CPR, and AED certifications, and be current in attaining the Continuing Education Units (CEUs) necessary for licensure.
Preferred:
- At least one year of previous experience in a long‑term care setting.
- Some education beyond the minimum nursing licensure level, including related training in supervision and infection control.
