Cardiovascular Nurse Educator

As a Nurse B Patient Educator, you will report to the Chronic Disease Management manager. Your responsibilities include delivering clinical education, facilitating self-management support for patients and families, and collaborating with an interdisciplinary team. This role emphasizes a compassionate approach to adult education and cardiac disease management, utilizing your advanced assessment and teaching skills.
Key Responsibilities:
• Deliver education for inpatient and outpatient patients
• Support clinical and self-management for cardiovascular disease
• Conduct follow-up calls post discharge for individual support
• Collaborate with other departments within SHA
• Provide staff education regarding cardiac disease
Requirements:
• 1 year experience in adult education or group facilitation
• 3 years clinical experience in cardiac care
• Bachelor of Science in Nursing (BScN)
• Registered/eligible with CRNS
• Valid Class 5 driver's license
