As a Registered Nurse at Center Well Home Health, you will play a pivotal role in providing expert, personalized care that empowers patients to regain their independence right lab orating with a dedicated team of healthcare providers, you'll implement care plans that facilitate recovery and help patients return to the life they cherish.

• Deliver skilled nursing care directly within patients' homes, including medication administration, wound care, and comprehensive assessments.
• Collaborate with physicians and healthcare professionals to develop and execute tailored care plans.
• Continuously monitor patient conditions and promptly report any changes.
• Educate patients and their families about disease management and treatment options.
• Keep thorough records of patient care and coordinate effectively with other healthcare team members.
• Regularly report patient progress and changes to the Clinical Manager and attending physicians.
• Supervise the work of Home Health Aides to ensure accurate documentation and care delivery.

• Experience in med-surg, ICU, ER, or acute care settings.
• Current and unrestricted Registered Nurse licensure.
• Valid CPR certification.
• Proven ability to work collaboratively within a healthcare team.
• Valid driver's license, auto insurance, and reliable transportation.

• Minimum of one year of nursing experience.
• Experience in home health care.

Pay is structured at $63.00 - $88.00 per visit/unit, with an annual base pay ranging from $92,600 - $127,400.
