- 3 Day a week scheduling
- 1:1 Elder Care
- Shift Rounds
- Current California LVN license.
- 1+ years experience in home health, hospice, or in‑home care preferred.
- 1+ years experience caring for adults with physical and cognitive disabilities.
- 1+ year experience providing catheter care, maintenance, monitoring, and infection prevention in a clinical or home health setting.
- Compassionate, dependable, and patient‑focused.
- Basic Life Support (BLS) certification required.
- Ability to work flexible hours, including evenings and weekends.
- Reliable transportation and valid driver’s license.
- Perform and manage catheter care, including maintenance, monitoring, and infection prevention.
- Provide skilled nursing care in patients’ homes across pediatric, adult, and geriatric populations.
- Administer medications, treatments, and therapies as prescribed by the physician.
- Monitor and assess patient health status, vital signs, and clinical progress.
- Collaborate with patients, families, and interdisciplinary care teams to develop and implement individualized care plans.
- Educate patients and families on disease processes, medication management, catheter care, and home care techniques.
- Respond promptly and professionally to changes in patient condition and emergency situations.
- Maintain accurate, timely documentation in electronic medical records (EMR).
- Participate in ongoing training and professional development to maintain and enhance clinical competency.
