• 2:30 PM to 11:00 PM
• 3 Day a week scheduling
• 1:1 Elder Care
• Shift Rounds

• Current California LVN license.
• 1+ years experience in home health, hospice, or in‑home care preferred.
• 1+ years experience caring for adults with physical and cognitive disabilities.
• 1+ year experience providing catheter care, maintenance, monitoring, and infection prevention in a clinical or home health setting.
• Compassionate, dependable, and patient‑focused.
• Basic Life Support (BLS) certification required.
• Ability to work flexible hours, including evenings and weekends.
• Reliable transportation and valid driver’s license.

• Perform and manage catheter care, including maintenance, monitoring, and infection prevention.
• Provide skilled nursing care in patients’ homes across pediatric, adult, and geriatric populations.
• Administer medications, treatments, and therapies as prescribed by the physician.
• Monitor and assess patient health status, vital signs, and clinical progress.
• Collaborate with patients, families, and interdisciplinary care teams to develop and implement individualized care plans.
• Educate patients and families on disease processes, medication management, catheter care, and home care techniques.
• Respond promptly and professionally to changes in patient condition and emergency situations.
• Maintain accurate, timely documentation in electronic medical records (EMR).
• Participate in ongoing training and professional development to maintain and enhance clinical competency.
