MENTOR Adult Day Health, a member of the Sevita family, provides day programs for elders with complex medical or rehabilitative needs. This allows seniors to obtain quality healthcare while remaining active and involved in the community and continuing to live in their own homes. Our programs are staffed with qualified, experienced licensed nurses and caregivers who are trained to provide individual assistance to each person we serve.

Licensed Practical Nurse / Licensed Vocational Nurse (LPN/LVN)
📍 Float for Springfield MA region
💲 $36/hour + mileage
🗓️ Full Time-Monday – Friday, Day Shift | No nights or weekend

What You’ll Do

  • Monitor health conditions and administer medications
  • Provide treatments and personal care services
  • Complete assessments and update care plans
  • Train staff on safety and infection control
  • Communicate with families, providers, and community partners

What We’re Looking For

  • Current LPN/LVN license in MA
  • At least 1 year of nursing experience preferred
  • Valid driver’s license and auto insurance
  • Strong communication and teamwork skills
