Connective is seeking a compassionate Outreach Worker to support clients' adjustment to community life. This position, ideal for individuals with a Diploma in Social Services, includes roles focusing on housing, nutrition, and lifestyle support. You will work with clients to set measurable goals and collaborate with professionals to facilitate resources for their wellbeing.
Key Responsibilities:
• Assess needs through client interaction and background research
• Develop intervention plans with clients for personal goals
• Support clients in various challenging environments
• Collaborate with families and organizations for service alignment
• Facilitate daily living support, education, and employment access
Requirements:
• Diploma in Social Services with relevant experience preferred
• Professional experience with vulnerable populations
• Ability to respond effectively in crisis situations
• Valid Class 5 BC driver's license
• Complete Vulnerable Persons Criminal Records Check
