About the Role
We are seeking a compassionate and detail-oriented Facility Nurse to join our team in a Minnesota 245D-licensed program. This role is essential in supporting individuals with disabilities by ensuring high-quality healthcare services, regulatory compliance, and person-centered care.
Key Responsibilities- Provide and oversee nursing services in accordance with Minnesota 245D Home and Community-Based Services (HCBS) standards
- Complete health assessments and develop individualized health service plans
- Administer medications and treatments as prescribed
- Delegate nursing tasks to unlicensed staff
- Train and supervise direct support staff on medication administration and health-related procedures
- Monitor and document health status, incidents, and progress notes
- Coordinate care with physicians, case managers, and interdisciplinary teams
- Ensure compliance with DHS licensing requirements and maintain accurate documentation
- Participate in audits, quality assurance, and program improvement initiatives
- Current Minnesota RN or LPN license in good standing
- Experience working with individuals with developmental disabilities or in a 245D setting preferred
- Knowledge of MN DHS 245D regulations and HCBS requirements preferred
- Strong assessment, documentation, and communication skills
- Ability to delegate, train, and supervise staff effectively
- CPR/First Aid certification (or willingness to obtain)
