Advanced Practice Provider – Nurse Practitioner or Physician Assistant

Concerto Care is seeking an Advanced Practice Provider (Nurse Practitioner or Physician Assistant) to be a core member of a multi‑disciplinary Field Team that delivers home‑ and community‑based care to patients with complex needs. This role involves providing compassionate, individualized, and holistic patient care—including urgent and chronic condition management—in a value‑based care delivery model.

Responsibilities
  • Provision of compassionate, individualized, and holistic patient care including comprehensive care, urgent care, and chronic condition management.
  • Collaboration with the multidisciplinary team to deliver transitional care support, social determinants of health interventions, behavioral health evaluation and management, and complex medication management.
  • Delivery of exceptional clinical care both in‑person and virtually, with in‑person settings including private homes, congregate living facilities, ambulatory clinics, and/or skilled nursing facilities.
  • Integration of clinical practices, data dashboards, and operational processes that support achievement of company performance goals into daily clinical approaches and workflows.
  • Promotion and participation in patient engagement and experience initiatives.
  • Maintenance of comprehensive medical records including timely and accurate completion of clinical documentation and coding, medication reconciliation, and recording of patient care‑related activities.
  • Active participation in care team huddles, operational huddles, organizational meetings, and clinical partnership meetings as requested.
  • Collaborative engagement with internal care team members and external providers to support care coordination including goals of care.
  • Adherence to medical policies, protocols, criteria, and clinical guidelines.
  • Adherence to compliance policies, procedures, and standards of conduct including all applicable laws and regulations.
  • Supportive function as growth/engagement ambassador to initiate and sustain relationships with patients, caregivers, facility staff, and clinical partners.
  • Participation in after‑hours on‑call schedule and other clinical care cross‑coverage as requested.
  • Other market‑specific duties as assigned.
Qualifications
  • Graduate of an accredited health professions school and successful completion of ANCC or AANP certification (or board‑certified Physician Assistant). PMHNP or NNP certifications alone are not eligible.
  • Active licensure in applicable state(s).
  • Active DEA licensure and state‑specific controlled substance registration, as applicable.
  • Advanced training in geriatrics and/or palliative medicine preferred.
  • Certification in basic life support (BLS) required.
  • Clinical experience caring for aging adults and adults with complex care needs required.
  • Clinical experience caring for patients in the home setting preferred.
  • Previous experience with managed care and/or value‑based healthcare delivery preferred.
  • Average to advanced computer and software skills required.
  • Ability and means to travel as needed; proof of liability and property damage insurance on vehicle used is required.
  • Driver requirements: licensed for a minimum of 5 years, no vehicle‑related suspensions, DUI, reckless driving, leaving the scene of an accident, vehicular manslaughter, felony convictions, or 3 or more moving violations within the last 5 years; no more than 2 moving violations or 1 at‑fault accident in the last 3 years.

Compensation

Base salary range $130,000 to $160,000 plus annual bonus (may fall outside the range shown). Total rewards package includes full healthcare coverage, a 401(k) with match, and other benefits.
