Under the strategic direction of the Manager, Health Services, the Primary Care Physician (Physician) functions interdependently with staff to provide shared, client-centered care that emphasizes health promotion, disease prevention and client education. The Primary Care Physician collaborates with Nurse Practitioners, Allied Health Providers, and community-based services and agencies to provide primary health care for our clients. The Primary Care Physician practices according to the Standards of Practice of the College of Physicians and Surgeons of Ontario (CPSO) and to guidelines and policies established at Vibrant.
In collaboration with Sunnybrook Health Sciences Centre Department of Family and Community Medicine, we are looking for a Physician with an interest in medical education. This position will include regular supervision of residents and medical students.

Summary of Duties- Assumes the primary care provider role in areas of acute and chronic disease prevention and management, consulting with the interprofessional team when necessary.
- Performs assessment, diagnosis, counselling, screening, referral, education, treatment and other medical functions as required according to the College of Physicians and Surgeons of Ontario standards of practice.
- Provides primary care to the general community with an emphasis on Vibrant Community Health’s priority populations.
- Maintains complete and accurate records of client visits including the data required by the Ministry of Health, utilizing the electronic health record.
