Position: Nurse Case Manager 2
Registered Nurse – East Metro Assertive Community Treatment Team (ACTT) (Full Time Permanent)

The Role and Opportunity
The Canadian Mental Health Association is looking for a Registered Nurse (RN) for the New Dimensions Assertive Community Treatment Team (ACTT) in the East Office. The Assertive Community Treatment Team (ACTT) is a multidisciplinary team providing community‑based support, rehabilitation and treatment for people living with mental illness. The team is based out of CMHA Toronto – Markham Site. Services are provided in the community and focus on stabilization, community integration, quality of life and recovery from mental illness.

Canadian Mental Health Association (CMHA) is the most established and extensive community mental health organization in Canada. Since 1918, CMHA has grown to provide mental health services and supports across Canada in over 330 communities. CMHA Toronto provides services to over 10,000 clients per year in 31 programs at 13 locations across Toronto.

Responsibilities
Reporting to the Program Manager, the Registered Nurse will work closely with the interprofessional ACT team to provide nursing care, health promotion, assistance with activities of daily living and support to individuals with serious and persistent mental illness in a community setting.

The RN will provide psycho‑education around medications and symptoms, assess mental status, and monitor physical health needs. The role requires developing a therapeutic alliance with clients and their support systems, working from a recovery‑based perspective, assessing client needs, linking to appropriate resources and monitoring treatment regimes. The RN will also support the agency’s primary health‑care and chronic disease‑management initiatives.

Skills You Bring
CMHA Toronto’s ideal candidate will be:
1) an undergraduate graduate in nursing and a Registered Nurse in good standing with the College of Nurses of Ontario;
2) have a minimum of two (2) years’ experience working with people with mental illness;
3) possess strong mental‑health assessment, medication monitoring, case management and crisis intervention skills;
4) have knowledge and understanding of the ACT model and recovery‑oriented practice; and
5) be solution‑focused, able to manage crisis, work with diverse communities and operate within an anti‑oppressive framework.

Experience with addictions, forensics and dual diagnosis is an asset. The candidate must hold a valid Ontario Driver’s License, have access to a vehicle and insurance coverage.

Salary: $68,975 – $75,581 per year.

35 per week.
