Program Overview – Housing Access Program, Housing & Homelessness Services Unit

The program’s goal is to support participants to increase their independent living skills within a variety of housing options, including program clients living in private rental housing. Within a client‑first approach, the following services are provided: behaviour modeling, crisis intervention, informal counselling, mediation with landlords and other tenants, referrals to appropriate community support services, assistance with maintaining access to rent supplements and income supports, and moving/retaining suitable independent housing.

Using comprehensive holistic support approaches, the Housing Support Worker engages with participants one‑to‑one to develop short and long‑term housing and support service activity plans to ensure that participants successfully obtain and maintain their housing and access any supports required to achieve their goals. Working with existing program partners—including the City of Toronto, Ontario Works, and the Ontario Disability Support Program as well as other community‑based partners—is an integral component to assisting participants secure access to necessary supports for independent living.

What

You Will Do

This position reports to the Housing Access, Partnerships & Tenant Supports in the Housing Unit. It is responsible for providing referral supports to individuals to obtain, maintain, and stabilise their housing in the Wood Green housing portfolio, including case management specifically for former residents of the Emergency Homelessness Project or Tent City.

Individual Support
  • Provide ongoing support to former Tent City participants to obtain and maintain their housing and connect them to a variety of community resources via home visits, drop‑in services, accompaniment to community resources, and scheduled appointments.
  • Respond and provide referral support during crisis situations to Wood Green Housing clients and other programs including Emergency Homelessness programs such as Tent City. Engage in tenant/landlord mediation to prevent evictions and relocations for assigned participants.
  • Work with participants to enhance their capacity to access community resources, including housing and support services, and develop effective problem‑solving strategies.
Facilitate Community and Program Linkages
  • Actively participate in each Program Steering and Services Committee, noting policy recommendations, identifying issues, and providing program updates.
  • Ensure ongoing collaboration with internal Wood Green units so that clients are connected to the organization’s integrated service model of employment, community health and social/community program services.
  • Develop and maintain working relationships with partnering agencies, including but not limited to the City of Toronto, Ontario Works, the Ontario Disability Support Program, other Housing Help Services, and community resources.
  • Ensure that client participants maintain eligibility for rent‑geared‑to‑income programs, including rent supplements and other housing programs.
  • Liaise with Ontario Works, the Ontario Disability Support Program, and other community resources to ensure participants have access to and maintain eligibility for income supports.
  • Develop and implement strategies with participants so they enhance their skills to access other community resources, supports, and housing options, working toward long‑term independence.
  • Communicate program policies, procedures, and guidelines to participants.
Program Development
  • Maintain accurate documentation, including client notes, case lists, and budget tracking, and assist with annual RGI reviews.
