Overview
We are seeking a dedicated and creative Diversion Case Manager to lead and expand our Shelter Diversion program. The role focuses on preventing homelessness for young people aged 16‑24 by identifying safe alternatives to shelter stays, coordinating with families, landlords, and community partners, and providing follow‑up case management to ensure housing stability.

Responsibilities

Engage with young people experiencing or at risk of homelessness to solve housing problems and secure safe alternatives.

Partner with families, friends and chosen supports to facilitate transitions.

Liaise and mediate with landlords and housing providers to prevent housing loss and promote retention.

Provide short‑term follow‑up case management services to maintain housing stability.

Distribute emergency funding to prevent loss of housing or secure new housing options.

Offer navigation support to shelters, drop‑in programs and other resources for those without diversion options.

Facilitate group activities, workshops and presentations as needed.

Collaborate with the Covenant House team, shelter system and broader community partners.

Work with stakeholders to develop and promote the specialized youth service.

Contribute to ongoing analysis, evaluation and quality improvement of the program.

Maintain accurate documentation, assessments, case notes and evaluation tools.

Qualifications

Minimum BSW or equivalent education and experience; MSW preferred.

Registered Social Worker in good standing with OCSWSSW preferred.

At least 3 years’ experience working with at‑risk or homeless youth.

Strong advocacy for homelessness prevention.

Excellent problem‑solving, communication and organizational skills.

Ability to work independently, proactively and collaboratively.

Trauma‑informed, harm‑reduction and anti‑oppressive practice knowledge.

Understanding of youth homelessness, 2

SLGBTQ identities, mental health, substance use and neurodiversity.

Knowledge of community resources relevant to at‑risk youth.
