SLGBTQ youth at Sherbourne as a full-time Case Manager (Youth Resource Worker). Help support young adults in achieving their goals through comprehensive case management. As a key member of the Community Programs team, you’ll perform intake assessments and provide tailored support to 2
SLGBTQ youth aged 14-29.
Your role will involve collaborating with various partners, developing Smart plans, and ensuring access to essential health services. Strong interpersonal skills and crisis management abilities will be vital.
Key Responsibilities:
* Conduct intake assessments and identify goals for service users
* Facilitate communication with service users and partners
* Provide brief counselling and urgent support as needed
* Organize workshops and outreach for group activities
* Maintain accurate records in the electronic medical system Requirements:
* Post-secondary education in social work
* Minimum 3 years experience with marginalized youth
* Knowledge of Electronic Medical Recording systems
* Strong problem-solving and professional communication skills
* Understanding of social determinants of health Become an advocate for 2
