Case Manager

Position: Case Manager at LOFT Community Services
Make a meaningful impact as a Case Manager at LOFT Community Services in Toronto, ON. This onsite role focuses on supporting clients facing mental health and substance use challenges.
As a Case Manager, you will coordinate essential support networks for clients overcoming mental illness and addiction. Your expertise will empower individuals to achieve greater independence in a structured and supportive environment. Fostering personal growth, you'll implement tailored recovery plans and provide crisis intervention services.

Key Responsibilities:

• Develop individualized support and recovery plans

• Promote personal empowerment through coaching and mentoring

• Establish a network of community and medical resources

• Provide crisis intervention and manage client emergencies

• Coordinate program events to foster client engagement
Requirements:

• Post-secondary education in human services

• At least three years of relevant experience

• Knowledge of mental health and addiction issues

• Strong interpersonal and communication skills

• Valid driver's license and reliable vehicle
