Grade: Band 6 (37.5 hours per week)
Reports to: Band 7 Renal Dietitian
Accountable to: Service Operational Manager for Integrated Therapies and Allied Health Professionals Service Manager

Summary of Post
As a Specialist Renal Dietitian you will be working with the supervision of the Band 7 Renal Dietitian to provide a specialist dietetic service to Low Clearance, Nephrology and Haemodialysis services. This includes the Renal Dialysis Unit, adult renal inpatients and outpatient clinics as required. Additional duties include:
 To deputise for Band 7 dietitians as agreed and to support Band 5 and other Band 6 dietitians and dietetic assistants
 Lead on audit, development and writing of clinical policies in own areas of work
 
 To provide clinical supervision and guidance to junior members of staff and student dietitians
 To provide support and training for a Band 5 renal rotation within the department
 Take an active part in the education and training of all levels of healthcare professional and medical staff, not exclusively within the Renal Department
 To provide expert dietetic and nutritional advice to healthcare professionals, patient carers, voluntary groups within and external to the organisation, not exclusive to renal dietetics
Main Duties and Responsibilities
Clinical
1. To be professionally responsible and legally accountable for the management of own clinical caseload and to organise this effectively with regard to clinical priority and effective use of time, seeking guidance from the Band 7 Dietitians for complex specialist cases when required.
2. To be responsible for working as an autonomous practitioner, requesting diagnostic tests, undertaking dietetic assessments and calculation of nutritional requirements based on anthropometry. This takes into account clinical condition, the interpretation of biochemistry and providing evidence based treatment regimes, including prescribing nutritional support within local and professional guidelines.
3. Understand and interpret monitoring and treatment of patients, e.g. biochemistry, medication, fluid balance and their effect on the dietetic management of the patient.
4. To work as an effective member of a multidisciplinary team: attend all relevant ward rounds, case conferences and multidisciplinary team meetings.
5. To provide highly developed communication and counselling skills to facilitate dietetic intervention. This entails skills of reflective listening, open questioning, empathy, explanation and reassurance to communicate dietary intervention to patients and carers to optimise nutritional outcomes. This may include terminally ill patients and patients with cognitive problems.
6. To be able to communicate complex information effectively with all patients, referral agents, other healthcare professionals and dietetic staff, to ensure the specific needs of patients are met. Through this to also gain valid consent to treatment and negotiate change with patients by breaking down barriers, using motivational skills, to enable them to achieve dietary targets set.
Professional and Supervisory Responsibilities
1. Responsible for clinical supervision of junior staff, including the Band 5 renal rotation and dietetic assistants.
2. Supervise work allocated to dietetic assistants.
3. To work co-operatively with other members of the Acute Dietetic Team to manage workload and maintain appropriate level of service during annual leave or other absence.
4. Participate where deemed appropriate by the Service Operational Manager for Integrated Therapies in the recruitment and selection of junior members of staff.
 
5. Initiate and participate in regular audit in own area of work. Participate in multidisciplinary research and collect data as required.
6. Evaluate and critically appraise research articles obtained using online databases on a regular basis.
7. Initiate and lead on implementing evidence based clinical practice, developing policies and clinical guidelines in own area of work.
8. Actively maintain own clinical professional development through self-directed learning to maintain up-to-date knowledge of clinical dietetic issues relevant to your area of practice.
9. Collect information and statistics relating to own area of work as required for service monitoring and evaluation via SystmOne.
10. Participate in identifying future dietetic service needs.
11. Collect data as part of the Dietetic Team for service audits and surveys (customer/patient satisfaction surveys), caseload management and individual service improvement audits.
12. Attend dietetic department meetings and take part in a rotating scheme to chair the meeting. Attend other relevant meetings in own area of work as required by the service.
13. Attend all multidisciplinary team meetings as relevant in specific area of work in order to provide assessment and treatment plans to other members of the multidisciplinary team.
14. Professionally responsible and accountable for keeping accurate and timely records in line with local and British Dietetic Association record keeping standards.
15. Maintaining a working understanding of the Trust’s Health and Safety and Clinical Governance policies and participate in Risk Assessment/Management activities within the service.
16. To follow Trust and Dietetic Department policies to ensure the provision of an effective and high quality service at all times.
17. To participate in the review and update the department’s information, diet sheets and standards in line with evidence-based practice.
18. At all times to maintain complete confidentiality of information.
Educational Responsibilities
1. To give lectures, presentations, seminars and to run workshops as requested by Service Operational Manager for Integrated Therapies or Band 7 dietitians including:
 In-service training for nursing, medical, AHP and other healthcare professionals as requested
 Teaching sessions for student nurses or return-to-practice groups
 Presentations, demonstrations and displays to patients, carers and members of public
2. To participate in training of student dietitians during their clinical placement and liaise with the Lead Placement Educator with regards to student training needs.
3. To develop and review educational material as requested by Service Operational Manager for Integrated Therapies or Band 7 dietitians.

Health and Safety Responsibilities
1. To ensure that the Trust’s Health and Safety Policies are understood and observed and that procedures are followed.
2. To ensure the appropriate use of equipment and facilities and that the working environment is maintained in good order.
3. To take the necessary precautions to safeguard the welfare and safety of yourself, patients, visitors and staff, in accordance with the Health and Safety at Work Act.
4. To undertake appropriate health and safety training to support safe working practice including, where appropriate, its management.
5. To demonstrate a practical understanding of risk assessment in relation to their areas of responsibility and to ensure safe systems of work are in place.
6. To ensure that all incidents occurring within the department are reported in accordance with Trust procedures, investigated and corrective action taken as necessary and/or reported to senior management and specialist advisers.
Moving and Handling Responsibilities
1. To ensure that all moving and handling tasks within their area of responsibility are assessed, that there is a manual handling plan indicating safe systems of work, and that documentation is regularly maintained and updated.
2. To observe and monitor the application of moving and handling skills, ensuring safe practice.
3. To attend appropriate training in accordance with the moving and handling assessed risks within the work area.
