About the RoleThe RN Case Manager (Float) plans, coordinates, and delivers hospice care across Shenandoah, Page, Warren, Rappahannock, and Fauquier Counties in VA. This role is responsible for comprehensive patient assessments, individualized care planning, and ongoing clinical support—ensuring patients and families receive compassionate, holistic care in their homes and communities.
You will serve as a key member of the interdisciplinary team, guiding care delivery, educating families, and coordinating resources to meet evolving needs throughout the hospice journey.
How You’ll Make a Difference Patient Assessment & Care Planning- Complete initial and ongoing comprehensive assessments of patients and families
- Develop, implement, and regularly update individualized Plans of Care
- Utilize clinical judgment to establish nursing diagnoses and appropriate interventions
- Collaborate with physicians and the interdisciplinary team to ensure aligned care delivery
- Deliver professional nursing care utilizing the full nursing process
- Administer medications and treatments in accordance with physician orders
- Support activities of daily living and promote patient comfort and dignity
- Maintain a safe and therapeutic environment for patients
- Provide education on disease progression, medications, and care techniques
- Counsel patients and caregivers to support emotional and physical needs
- Empower families with tools and guidance for effective caregiving
- Collaborate with the interdisciplinary team to address holistic care needs
- Maintain accurate, timely clinical documentation within the EMR
- Communicate changes in patient condition and coordinate care with physicians
- Ensure appropriate documentation of symptom management and outcomes
- Coordinate equipment, supplies, and services needed for patient care
- Partner with the hospice interdisciplinary group to provide coordinated care
- Participate in interdisciplinary team meetings
- Support continuity of care across settings and services
- Current, unrestricted Registered Nurse (RN) license in Virginia
- CPR Certification
- Ability to travel throughout Shenandoah, Page, Warren, Rappahannock, and Fauquier Counties
- Proficiency with electronic medical records and computer systems
- Strong verbal and written communication skills
- Bachelor of Science in Nursing (BSN)
- Experience in hospice, home health, hospital, LTC, or assisted living settings
- Knowledge of hospice philosophy, Medicare, insurance, and regulatory requirements
- Previous community health, education, or service experience
- Strong presentation and patient education skills
