In-Home Nurse Practitioner or Physician Assistant Part Time - Roanoke

Signify Health, part of CVS Health, is seeking a Part‑time Clinician (Nurse Practitioner or Physician Assistant) to provide In‑Home Health Evaluations, engaging with patients in the comfort of their homes and helping bridge gaps in care. The role involves building trusted relationships, delivering flexible, comprehensive and personalized evaluations at the patient’s front door.

The visit lasts about 45 minutes and includes a medication and medical history review, a physical evaluation, and additional diagnostic tests (e.g., diabetic eye exams, spirometry) if ordered by the health plan. Clinicians use an iPad and intuitive workflow; prescribing, lab ordering, or changing treatments is not required.

As an in‑home clinician, your primary workplace is the private residence of health‑plan members. The environments vary and may contain pets, secondhand smoke, and allergens or irritants. Candidates must be able to perform duties safely and comfortably in such settings.
