About the Role:
Provides direct clinical care of clients, including ordering of lab tests, counseling of patients, developing treatment plans and prescribing medication within the scope of their education and certification of the collaborative agreement with their collaborating physician as assigned at one or more clinical facilities in the Lake County Health Department and Community Health Center.
Hours: 40 hours a week, Monday - Friday (8:30am-5:00pm)
FLSA STATUS: Exempt
Collaborative agreement with appropriate physician, if not certified for independent practice in Illinois
Evaluates clients regarding their treatment needs related to addictions and withdrawal management, manages medical aspects of addictions and withdrawal management, and prescribes medication when appropriate.
Performs addictions-related psychiatric or medical evaluations, as requested by other staff members, for purposes of diagnosis and for treatment planning.
Consults with staff regarding clients not necessarily seen directly by the physician. This will include, but is not limited to, cases in which medical or psychiatric supervision is necessary for insurance purposes.
Conducts crisis intervention/evaluation/referral to persons who present themselves at the program.
Responds to inquiries, gives appropriate information and referrals, and documents, if necessary.
Provides direct services to clients including individual and group treatment.
Focuses on client empowerment in providing treatment, seeking to assist clients to build on strengths in addressing treatment needs.
Facilitates with clients and collaborates with the treatment team to develop, review and update treatment goals and referrals both internally and externally.
Provides education and direction to clients, family members and/or significant others.
Takes responsibility for training in and documenting clinical chart requirements to meet program and state standards.
Must meet minimum productivity expectations.
Maintains an awareness of and professional involvement/collaboration with community resources.
Ability to operate computers and knowledge of electronic health records.
Addiction Treatment Program (ATP) residential care specific responsibilities:
The Health Department’s main provider of medical care for clients with addiction problems seeking withdrawal management and residential treatment
Medical screening of all potential ATP candidates, as well as Crisis Care Program (CCP) as needed.
Intake examinations of all ATP withdrawal management (“Detox”) and rehabilitation clients, and of CCP clients as needed
Medical addiction treatment for ATP withdrawal management clients, including prescribing medication and monitoring progress.
Medical addiction treatment for ATP rehabilitation clients when appropriate, including prescribing Medication-Assisted Treatment (MAT) medications
Medical care of ATP and CCP clients in residential setting.
Coordinating care with ATP nurses and counselors, and with CCP staff when needed.
Coordination with ATP and CCP psychiatric care providers.
Substance Abuse Program (SAP) outpatient clinic specific responsibilities:
Evaluations of addictions-related treatment needs in outpatient clinic and Intensive Outpatient Program (IOP) settings.
Manages addictions through prescribing of Medicinal Addiction Treatment (MAT) when appropriate
Coordinating with SAP nursing staff in collection and evaluation of laboratory specimens and results
Coordinating with SAP nursing staff in administration of injectable medications
Coordinating with SAP staff in regarding all aspects of addictions-related patient care, in clinic and Intensive Outpatient Program (IOP) settings.
Education, Qualifications, and Certification:
Graduation prior to start date from a Master’s or Doctoral Degree in Nursing from a:
Psychiatric-Mental Health Nurse Practitioner (PMHNP) program accredited by either the Commission on Collegiate Nursing Education (CCNE) or the Accreditation Commission on Education for Nursing (ACEN), or
Family Nurse Practitioner FNP program accredited by either the CCNE or the ACEN, or
Adult Nurse Practitioner (ANP) program accredited by either the CCNE or the ACEN
At least 1 year of experience providing medication-assisted treatment (MAT) half-time, or equivalent
Certification and Licensure
Initial and continuing certification as a
PMHNP by the American Nurses Credentialing Center (ANCC) or other national certifying body as required by state law, or
FNP by the ANCC, the American Academy of Nurse Practitioners National Certification Board (AANPCB) or other national certifying body as required by state law, or
ANP by the ANCC, or other national certifying body as required by state law.
Current active and unrestricted license from the State of Illinois as an Advanced Practice Nurse; or a letter from the Illinois Department of Professional Regulation stating that the application for licensure endorsement has been approved, with receipt of Illinois license following within six months of initial application date for endorsement
Current active and unrestricted Controlled Substance Registration (CSR) from the State of Illinois
Registration with the Drug Enforcement Agency (DEA)
CARA Act waiver from the DEA.
