Licensed Practical Nurse (LPN) | Winchester Rehabilitation and Nursing Center
We are seeking a compassionate and dependable Licensed Practical Nurse (LPN) to join our skilled nursing team at Winchester Rehabilitation and Nursing Center in Winchester, MA. As an LPN at our 117‑bed skilled nursing facility, you will provide direct nursing care, supervise nursing support staff, and ensure high‑quality, resident‑centered care in accordance with facility policies and regulatory requirements.
Responsibilities- Supervise Certified Nursing Assistant (CNA) staff and other nursing personnel on assigned units
- Provide direct care, including medications, wound care, colostomy changes, tube feedings, suctioning, IV administration, and other skilled nursing interventions as assigned
- Observe, document, and report changes in residents’ status to physicians, families, and leadership
- Transcribe and carry out the physician's orders accurately
- Participate in admissions, discharges, and transfers
- Conduct rounds to ensure resident needs are met, and staff follow facility policies
- Schedule follow‑up appointments and coordinate transportation needs for residents
- Collaborate with the interdisciplinary care team to ensure holistic care
- Maintain adequate stock of medications, supplies, and equipment; notify appropriate personnel of needs
- Promote teamwork, mutual respect, and effective communication
- Participate in survey processes, inspections, and QAPI activities
- Treat all residents with dignity and respect, protecting residents’ rights and maintaining confidentiality
- Follow infection control policies, safety procedures, and life safety/emergency protocols
- Report any abuse, neglect, exposure incidents, or noncompliance according to policy
- Complete all assigned training and skills competency requirements
- Current, unencumbered Massachusetts LPN license
- Strong communication and critical‑thinking skills
- Ability to handle situations with tact and professionalism
- Prior long‑term care experience preferred but not required
