Qualifications for Nurse Case Manager- Current unencumbered State of Massachusetts RN license
- Knowledge of Medicare, Medicaid, PPS, PDPM, MDS and third-party payment
- Knowledge of regulatory agencies/licensure standards
- Must have minimum 5 years’ experience working in geriatrics with subacute experience
- Must have strong clinical background
- Case management experience strongly preferred
- Monitoring level of HMO patients on admission and throughout stay
- Obtaining prior approval for all HMO patients throughout the facility for transportation, therapy, etc.
- Tracking HMO and Medicare days
- Obtain pre-authorization as needed for special services and equipment
- Ensuring sound clinical and financial outcomes
