Creative Health Solutions is hiring a Speech Language Pathologist (SLP) for our Skilled Nursing Facility (SNF) in Winter Haven, Florida, providing both inpatient and outpatient therapy services.

- Evaluate and treat patients as an SLP in an SNF inpatient and outpatient setting
- Develop and implement individualized Speech Language Pathology treatment plans
- Collaborate with physicians, nursing, and rehab team members
- Maintain accurate documentation in compliance with Medicare and facility standards
- Educate patients and families to maximize functional outcomes and independence
- Work closely with a team of healthcare professionals to create personalized treatment plans for patients
- Document patient progress and communicate with healthcare professionals regarding treatment plans and patient outcomes
- Continuously educate patients and their families on therapeutic procedures and techniques
- Stay up to date with the latest therapy techniques and methodologies
- Active Speech Language Pathology (SLP) license in Florida (or ability to obtain – relocation support available)
- Experience in SNF, inpatient rehab, or outpatient physical therapy preferred (new graduates encouraged to apply – clinical mentoring provided)
- Strong communication and clinical decision‑making skills
- Team‑oriented mindset with a passion for patient care and growth
