Job Overview

The position), utilizing innovative and flexible solutions, program planning and best practices with an emphasis on standardization and high reliability, is responsible for establishing an environment that fosters teamwork, role competence and proficiency in the satellite clinical setting. This position works closely with leadership and a broad spectrum of healthcare providers and administrative personnel to ensure optimal patient care in the satellite clinical setting.

This position is housed off‑site in an affiliated organizational setting with the potential to flex on‑site as needed to meet patient care needs. This position works in close liaison with the Health Care Team to ensure that patient care is coordinated, comprehensive, and that care transitions are seamless. This position practices at the of their scope and actively develops, coordinates, oversees and administers care to patients in accordance within the framework of team‑based patient‑centered model of care.

Minimum Qualifications
  • 1. Bachelor’s degree in Nursing (BSN). Associates degree considered with review of Nurse Executive Council.
  • 2. Registered Nurse (RN) license.
  • 3. Basic Life Support (BLS) certification.
Preferred Qualifications
  • 1. Specialty nursing certifications.
Duties and Responsibilities
  • 1. Provides leadership within nursing by providing direction and priority setting in regards to patient care.
  • 2. Develops and maintains collaborative relationships between all members of the Health Care Team inclusive of administrative, clinical, research and inpatient partners thus producing high performing, patient focused teamwork.
  • 3. Acts as a resource for all staff in regards to education, planning, relocating and development of evidence-based, best practice clinical programs pertaining to the specialty or primary care patient population.
  • 4. Mentors and actively participates in overseeing all clinical support staffing in the ambulatory clinical setting; delegates responsibilities appropriately and within scope.
  • 5. In conjunction with satellite clinical and administrative leadership, participates in coordinating any transition of patient care from the satellite setting to another in collaboration with and maintaining open communication with a wide range of interdisciplinary team members with the goal of achieving an exceptional patient experience.
  • 6. Identifies high risk patients and works with the appropriate services and interdisciplinary team members to achieve the best possible patient care and outcomes.
  • 7. Promotes shared decision making with patients in regards to healthcare; educates the patient and /or family regarding the patient’s clinical condition, treatment, and their role in their healthcare decisions.
  • 8. Utilizes a variety of telecommunications technologies to assess, triage, provide nursing consultation, and perform follow‑up and surveillance of patient status and outcomes.
  • 9. Collaborates and actively engages with the care team and practice leadership to identify opportunities and solutions to improve quality, patient access and cost savings.
  • 10. Demonstrates compliance with regulatory requirements i.e. MIPS, fall assessment.
  • 11. Participates in satellite quality and safety initiatives as indicated.
Physical Requirements
  • 1. Location is off‑site in an affiliated satellite location.
  • 2. Frequent travel may be required from satellite clinic to main hospital.
  • 3. Ability to oversee multiple issues/ situations simultaneously with an emphasis on safety.
  • 4. Prolonged, extensive, or considerable standing/walking.
  • 5. Lifts, positions, pushes and/or transfer patients and equipment.
  • 6. Considerable reaching, stooping, bending, kneeling, crouching.
  • 7. Frequent exposure to hazardous chemicals, sick patients, bodily substances, noise and possible exposure to radiation, lasers, electric shock, etc.
  • 8. Regularly exposed to the risk of blood borne diseases and other transmissible infections.
  • 9. Contact with patients under wide variety of circumstances.
  • 10. Subject to varying and unpredictable situations.
