- Lead and collaborate to provide management and delivery of patient care plans as a home health registered nurse case manager.
- Deliver quality patient care in accordance with care plans.
- Comply with charting and documentation deadlines.
- Self‑manage time and schedule; travel approximately 20% of the day to patient locations.
- Utilize tablets, mobile phones, and EMR software for patient care.
- RN license in the state of employment.
- Graduate of an approved school of practical nursing.
- Minimum one year of RN experience with demonstrated clinical competence.
- Current driver’s license.
- Demonstrated experience with tablets, mobile phones, and EMR software.

Low end of the range reflects the average annual compensation earned based on a fully productive caseload; high end reflects those who exceed productivity. Compensation potential varies by market.
