The main services provided by the Embassy include:

In case of a lost or stolen Passport:

  • Form A (duly completed)/Form B (for person below 12 years)
  • Three (3) passport size photographs which should not be older than 6 months
  • Duly filled in Form - Report of Lost Passport
  • Complete Memorandum
  • Original Pakistan National ID Card, CNIC/NICOP/POC (along with one photocopy)
  • Police report (original)
  • Photocopy of the lost Pakistani passport
  • Documentary evidence showing proof of Pakistani nationality i.e. citizenship certificate, Domicile certificate issued by the Government of Pakistan

NOTE: Issuance of passport in such cases is subject to verification from original issuing authority. Third time lost:

  1. Pakistan's Mission Abroad will issue emergency passport for return to Pakistan with an intimation to Dte Gen I&P.
  2. In Pakistan, the case will be referred to Dte Gen I&P for further action

Transit visa is issued to those who are travelling to other countries while transiting Pakistan.     Applicants should produce the original air ticket up to the country of final destination including visas of destination country. Transit visa is valid for 72 hours.

When applying for a Visa one should note the following:

  • Passport should be valid for six months from travel date.    
  • No column in the visa application form should be left blank. Incomplete forms will not be accepted. Supportive documents should be submitted, where indicated.     
  • Visa is liable to be cancelled if information requested in the form is withheld or found to be incorrect.    
  • Proof of financial support may be required (Consular Section will inform the applicant when required).    
  • Applicants could be asked to appear for an interview.    
  • Nationals of other countries should apply for visa from their country of origin.    
  • Visitors to Czech Republic are not entertained for grant of Visas.    
  • Visa fee is subject to periodical revision without notice.    
  • Visa fee is charged per person and not per passport.    
  • Fee is only payable in cash against certified Embassy receipt (Please ensure that you have the original receipt for return of documents).   
  • Personal cheques are not accepted    
  • Submitted application and documents cannot be returned.
