If Polish citizen loses his/her passport, he/she is obliged to immediately notify the passport authority which issued the passport. If the passport is lost abroad, Polish Embassy or Consulate must be contacted.

Citizens of China need tourist visas to visit Poland.

Documents required:

  • Application form.
  • 1 passport size photograph.
  • Original passport.
  • Copy of the passport.
  • Copy of ID card.
  • Hukou and its copy.
  • Travel health insurance and its copy.
  • Documents proving the purpose of the journey.
  • Confirmation of sufficient financial means.
  • Confirmation of employment/private business/studies.
  • Round-trip flight ticket with fixed dates (reservation only).
  • Hotel reservation or other proof of accommodation.
  • Residence permit (for residents holding non-Chinese passport only).
  • Consent from custodian, certificate of custody, letter from school (if only child is applying and parents/custodian(s) is/are not present). 
  • Travel agency’s license and covering letter (Travel agency must present its license and the covering letter specifying the list of the passengers, itinerary, the name of partner travel agency/co-organizer in Poland as well as the plan of the trip).

The Embassy is closed on Chinese and Polish Public Holidays.
