If Polish citizen loses his/her passport, he/she is obliged to immediately notify the passport authority which issued the passport. If the passport is lost abroad, Polish Embassy or Consulate must be contacted.

Russian citizens require a visa to visit Poland. Visa requirements are:

  • Visa application form.
  • Travel document and a copy of the first page of the passport.
  • Two color photographs.
  • Visa payment.
  • Medical insurance.
  • Copy of the internal passport.
  • If the applicant is a minor: birth certificate (original and photocopy); consent of both parents/guardians, if a minor is traveling alone, or consent of the other parent/guardian if the minor is traveling with one parent/guardian; copies of valid Schengen visas of the parents/guardians traveling along with a minor.
  • Proof of legal residence in the Russian Federation, if the applicant is a citizen of a third country.

Consular department accepts payments for consular services in euros. Visa application fee is non-refundable.
