If Polish citizen loses his/her passport, he/she is obliged to immediately notify the passport authority which issued the passport. If the passport is lost abroad, Polish Embassy or Consulate must be contacted.
The Embassy of the Republic of Poland in Ankara and The Consulate General of the Republic of Poland in Istanbul are pleased to announce the Visa Application Centre operated by VFS in Ankara, Istanbul, Izmir, Gaziantep and Antalya. This service is available for Turkish residents/other country nationals legally residing in Turkey who wish to apply for a Schengen or long stay visa to Poland.
All applications collected by the Visa Application Centers will be sent to The Embassy of the Republic of Poland in Ankara / The Consulate General of the Republic of Poland in Istanbul for assessment and resolution. Visa Application Centre staff have no say in the visa decisions taken by the Embassy or the Consulate General.
Documents requested at the visa application:
- Application form completely filled (in Polish, English or Turkish) and signed by the applicant.
- The separate application for each child with photograph in case of a journey with a child being registered in parent's passport.
- Two biometric photos (no more than 6-months old, 35-40mm in width, close up of your head and top of your shoulders so that your face takes up 70-80% of the photograph, in sharp focus and clear, of high quality with no ink marks or creases, show you looking directly at the camera, show your skin tones naturally, have appropriate brightness and contrast, be printed on high quality paper and at high resolution, show your eyes open and clearly visible - no hair across your eyes, show you facing square on to the camera, not looking over one shoulder (portrait style) or tilted, showing both edges of your face clearly, be taken with a plain light-colored background).
- Original passport issued within the previous 10 years and valid at least 3 months beyond the end of the intended visit, that must contain 2 blank pages to ensure that there is space to affix entry/exit stamps for visa holder.
- Proof of adequate travel medical insurance covering the entire period of intended stay, valid for all Schengen States and covering any expenses which might arise in connection with repatriation for medical reasons, urgency medical attention, emergency hospital treatment or death during the stay. The minimum coverage must be 30.000 EUR.
- Documents based on applicant's occupation.
- Flight/travel reservations or other proof of intended transport,
- Evidence of hotel bookings or travel plan,
- Proof of means of subsistence: e.g. bank account statement on the account turnover during the last three months (stamped and signed by the bank ), and proof of regular income e.g. salary slips of the last three months (or similar).
