Theft or loss of a valid passport must be reported to the police and a police report should be presented when applying for a new passport. Otherwise a passport is applied as normally. When reporting a lost passport to the police it should be noted that a report made by telephone and a registration number given on the basis of that might not be adequate proof to the insurance company. The report can be made in person at a police station so that a report of the crime is available in writing.
The Consular Section of the Polish Embassy in London informs that from 14th August 2005 the Republic of Poland abolished the requirement of diplomatic or consular legalisation for foreign documents under the provisions of the 05th October 1961 Hague Convention. The present Convention shall apply to public documents which have been executed in the territory of the United Kingdom of Great Britain and Northern Ireland and which have to be produced in the territory of the Republic of Poland.
Please note that Poland does not recognize the Certificate of Travel (Black) and Certificate of Identity Document (Brown) issued by the Home Office. The applications for a Schengen visas are only accepted from persons who have valid residence permit and reside in the United Kingdom. The UK residence permit should be valid for at least three months after the expiry date of your Schengen visa. If you do not hold residence permit in the UK, you need to make an application in the country of your origin. Please note, the applications from holders of UK tourist/visit visas (type C) will not be accepted.
The visa fee is 51 GBP for all categories of Schengen visa. The payment must be made on the day of submitting an application. The Embassy accepts cash, postal order or cheque (made out to the Embassy of the Republic of Poland) payment. No refund of the fee is available even if the visa is rejected.
