Theft or loss of Slovak passport should be reported to local police who will issue a written confirmation of theft/loss. Loss of travel documents shall be reported at the Embassy in person.

List of supporting documents to be submitted by applicants in China for short stay Schengen visas:

  • Valid passport with at least 2 empty pages (validity must exceed return date by at least 6 months) together with one copy of the information page of the passport
  • Completed and signed Schengen visa application form.
  • 2 photographs in color (3,5 x 4,5 cm) on white background.
  • If applicable, copy of previously granted Schengen visa (also visa of United Kingdom, USA and Canada).
  • Travel insurance valid for the entire duration of intended stay in Schengen area with the coverage of minimum 30.000 EUR.
  • Copy of all pages of "Hukou" (no translation needed).
  • Return flight ticket reservation (only reservation at this stage, the actual ticket should be purchased only after visa being granted).
  • Proof of accommodation in Slovakia for the whole duration of intended stay.
  • Travel programme - documents providing clear evidence about the applicant's program in Slovakia (transportation booking, itinerary, etc.).
  • Proof of solvency - documents confirming financial resources for stay in Slovakia (bank statements from the last 3 months, no deposits).
  • Additional documentation for minors.

As from 12 October 2015 and introduction of the Visa Information System (VIS), all applicants will be required to provide their biometric data (10 fingerprints and digital photograph) when applying for Schengen visa (together with an application form and standard set of supporting documents).
