Requirements for applying for a new passport:

• Duly filled passport application form – ( can be obtained from this Mission )
• Current Passport
• Four passport size (4.5cmx 3.5cm) color photographs
• Original Birth Certificate ( translations are not accepted)
• If the applicant was born outside of Sri Lanka, the Citizenship Certificate
• If the applicant is a citizen by registration, the Citizenship Certificate.
• National Identity Card issued by the Commissioner for Registration of Persons
• Copy of the Visa of country of residence

Issuance of New Passports in lieu of Lost Passports:

• Duly filled passport application
• Four passport size (4.5cmx 3.5cm) photographs
• A Police report obtained from a local Police station
• A copy of the previous passport
• Original Birth Certificate (translations are not accepted)
• National Identity Card issued by the Department of Registration of Persons
• Valid visa of applicant of country of residence
• Application fee

NOTE: For applicants not holding a valid visa will be issued a Non Machine Readable Passport (NMRP), which will be valid only to return to Sri Lanka. Every applicant, including children, requesting a NMRP should be present for an interview with all relevant documents at the Consular Division of the Embassy

A Temporary passport can be issued in lieu of a lost passport of a Sri Lankan national, only to return to Sri Lanka.

Child entry inclusion in parent passport only can be done in 'M' series Sri Lanka passports. If you have been issued a passport beginning with "N" series, please note that the child included passport will be cancelled upon arrival in Sri Lanka.

Sri Lankan Citizens who are granted with the Austria or accredited country’s Citizenship, have to forward their application forms along with following documents with two set of photo copies in order to renounce the Sri Lanka Citizenship.
