Overview
Licensed Practical Nurse, Hemodialysis - ACDU has a Relief Part Time position with Fraser Health’s Renal Services team at Abbotsford Community Dialysis Unit, Abbotsford, BC. This role requires post-graduate specialty education in Nephrology. Fraser Health offers sponsorship for LPNs to gain specialty education through an in-house BC Renal program. If you have not completed a post-graduate program in Nephrology but are interested, please apply and indicate your interest in pursuing Specialty Education.

Responsibilities- Assesses, plans, implements, and evaluates patient/client care during pre-dialysis, intra-dialysis and post-dialysis periods, maintains therapeutic relationships, and respects patient/client autonomy in decision-making and care planning.
- Contributes to the development and ongoing changes to the individualized plan of care; participates in multidisciplinary care planning meetings and family conferences; collaborates with the RN and other nephrology team members; escalates care as needed.
- Plans and prioritizes client care; responds to unexpected events within scope of practice; performs hemodialysis nursing duties in accordance with LPN competencies.
- Facilitates patient teaching to improve understanding of care plans and chronic disease management; revises teaching strategies in collaboration with the interdisciplinary team.
- Documents care, monitors outcomes, administers medications and/or provides education as per policies; participates in quality improvement activities.
- Maintains a safe environment by following safety, infection control, and emergency procedures; reports unsafe conditions or malfunctioning equipment.
- Supports orientation and education of new staff and students; performs other related duties as assigned.
- Grade 12, graduation from an approved Practical Nursing program, and successful completion of post-basic education through an in-house BC Renal-affiliated program; plus one (1) year recent related experience as an LPN in acute care, or an equivalent combination of education, training and experience.
- Current full practicing registration with the British Columbia College of Nurses and Midwives (BCCNM); current CPR Level C or equivalent and First Aid certificates.
- Effective communication skills (verbal and written).
- Physical ability to perform duties; able to work independently and in collaboration with others.
- Ability to operate related equipment; organize and prioritize workload; establish rapport with clients; observe and respond to changes in clients’ conditions.
