The salary range for this position is CAD $41.42 - $59.52 / hour

We are looking for Registered Nurses, Primary Care to work on a casual on-call basis in a number of our Urgent Primary Care Centres at the locations listed below.
- Abbotsford - Urgent Primary Care Centre
- Mission - Urgent Primary Care Centre
- Chilliwack - Urgent Primary Care Centre

Responsibilities- Provides direct nursing care to clients receiving scheduled or urgent/timely primary care; assesses clients and collects information from various sources; makes a nursing diagnosis and determines appropriate interventions or collaborations.
- Processes physician or nurse practitioner orders by initiating standing orders, administering medications, carrying out treatments and monitoring effects.
- Establishes a therapeutic relationship with clients to ensure autonomy, dignity and privacy in decision-making and care planning.
- Screen referrals, provide individualized assessments, inter-professional care planning and interventions; facilitate access to timely and appropriate health services; coordinate with primary care providers and family supports.
- Develops comprehensive shared care plans in collaboration with the inter-professional team and transitions care to primary/community providers or community agencies.
- Delivers direct planned and unplanned client care, explains care to clients and families, and ensures understanding of the care plan.
- Reviews care plans with the inter-professional team to determine timing and referrals, and supports transitions across the care continuum.
- Discusses clinical findings with clients/families and develops action plans and goals; uses system information to inform care plans and prioritizes care needs.
- Supports clients and families in navigating the health care system; advocates for client choices and transitions to other programs or providers.
- Outreach nursing to clients in home or alternative settings; assesses status, plans care, and advises on community resources.
- Provides health counselling and education to promote self-management and wellness; evaluates learning outcomes.
- Maintains clinical records and statistical information in accordance with policies; supports professional practice growth.
- Participates in quality improvement, risk management, and education of nursing staff and students as appropriate.
- Contributes to the development and revision of standards of care and participates in quality improvement initiatives.
- Performs other related duties as assigned.
Education and Experience
Current practicing registration as a Registered Nurse with the British Columbia College of Nurses and Midwives (BCCNM). One (1) year of recent related clinical experience in acute care, urgent/emergency care or a related area, including assessment of children, youth and adults; or an equivalent combination of education, training and experience. Valid BC Driver’s license and access to a personal vehicle for business purposes, as required.

Demonstrated ability to complete initial and ongoing client assessments and provide nursing care; effective communication (verbal and written); ability to integrate data from multiple sources; knowledge of community resources and continuum of care; ability to promote client-focused care with cultural sensitivity; ability to prioritize and manage multiple clients; ability to educate clients and others; ability to work in dynamic environments and as part of an interprofessional team;
capacity to operate related software; physical ability to perform duties.
