Job SummaryBring your clinical expertise and passion for providing quality, compassionate care to a challenging and rewarding role as a Nurse Practitioner in the Heart Function Clinic located in Abbotsford Regional Hospital, Abbottsford, B.C. The Abbotsford Regional Hospital Heart Function Clinic (ARH HFC) is an outpatient clinic where a multidisciplinary team manages patients with a diagnosis of heart failure or suspected heart failure with or without low ejection fraction using evidence‑based care.
These patients have complex medical needs and will require patient education and optimization of therapy through medications over multiple visits. Once optimized, patients are returned to their primary care providers or specialists for ongoing care. The Nurse Practitioner will work with the point‑of‑care Heart Health and Cardiology team, which comprises heart failure specialists, Nurse Practitioners, registered nurses, dietitians, pharmacists, and unit clerks.
We have an exciting opportunity for a Nurse Practitioner to join our Heart Function Clinic Team at Abbotsford Regional Hospital. As part of this rewarding role, the NP works collaboratively within a multidisciplinary team with a coordinated approach to ensure safe, quality, effective, and timely care for patients. The NP role contributes to patient care through advanced NP clinical practice, education, consultation, research, leadership, and professional practice.
This is a great opportunity for a new graduate Nurse Practitioner to join a well supported fellowship program to help consolidate clinical skills. You will be welcomed into a new graduate NP community of practice team helping to support your successful transition into independent practice. Be part of our clinical enhancement program with regular in‑house skills development such as 12‑lead ECG interpretation, point‑of‑care ultrasound (POCUS), suturing and x‑ray interpretation in addition to receiving centralized NP orientation.

The Nurse Practitioner is responsible and accountable for the comprehensive assessment of patients/clients/residents including diagnosing diseases, disorders, and conditions. The Nurse Practitioner initiates treatment, including healthcare management, therapeutic interventions and prescribes medications in accordance with statutory and regulatory standards, limits, and conditions, and employer policies and procedures. The Nurse Practitioner provides professional guidance to other health professionals and practices autonomously and interdependently within the context of an interdisciplinary healthcare team, making referrals to specialist physicians and others as appropriate.
The position collaborates with patients/clients/residents and other health professionals to identify and assess trends and patterns that have implications for patients/clients/residents, families and communities; develops and implements population and evidence‑based strategies to improve health and participates in policy‑making activities that influence health services and practices. The position participates in peer review and self‑review to evaluate the outcome of services at the patient/client/resident, community and population level.
