In accordance with the British Columbia College of Nurses and Midwives (BCCNM) standards of practice and the Mission and Values of Fraser Health, and working collaboratively with the interprofessional care team, the Registered Nurse provides direct patient care by assessing, planning, providing and evaluating nursing care to acutely ill elderly patients.
Responsibilities- Assess patients within an acute care unit for elders; collect information from various sources using observation, communication, data review and physical assessment. Make a nursing diagnosis and plan, perform, and evaluate nursing interventions and care.
- Collaborate with patients and families/caregivers in developing the care plan; establish a therapeutic relationship that respects privacy, dignity, and patient autonomy.
- Participate in admission care and discharge planning; update and implement care plans in collaboration with the interprofessional care team.
- Coordinate nursing care; set priorities, assign and delegate tasks to support staff, and make referrals to the interprofessional care team per standards.
- Collaborate with Physicians, Licensed Practical Nurses, Clinical Nurse Educators, and other health care professionals to adjust care plans as needed and respond to changes in patient condition.
- Educate patients and families about illness prevention, health restoration and maintenance; assess learning needs and provide information to support self-care and discharge planning.
- Discuss referral information with patients and families; initiate referrals to appropriate health care/community agencies in collaboration with the care team.
- Document assessments, diagnoses, observations, interventions, patient responses, outcomes, and referrals per practice standards and Fraser Health procedures.
- Participate in nursing committees, care conferences and quality improvement activities as assigned; engage in ongoing education to maintain clinical competence.
- Serve as a preceptor or mentor to new nursing staff and students; contribute to orientation and knowledge sharing.
- Provide feedback to the Manager on patient care programs, tools, and equipment; perform other related duties as required.
Education and Experience
Graduation from an approved school of Nursing with current practicing registration as a Registered Nurse with the British Columbia College of Nurses and Midwives (BCCNM). One (1) year of recent related acute care experience, or an equivalent combination of education, training and experience.
Skills and Abilities- Ability to conduct initial and ongoing patient assessments and provide nursing care with appropriate technical and safety interventions.
- Effective communication with patients, families/caregivers, the public, medical staff and the interprofessional care team.
- Ability to promote person-centered care with empathy, cultural sensitivity and patient advocacy.
- Ability to analyze data from multiple sources to problem-solve; teach patients and families about geriatric care topics.
- Strong organizational and prioritization skills; ability to collaborate with the interprofessional care team.
- Use of evidence-informed practice to support nursing decisions; operate related equipment and computer applications.
- Physical ability to perform the duties of the position.
