We are looking for a motivated and skilled practitioner who possesses the ability to carry out planned nursing care assessment and reviews. The practitioner will work as part of a team and be able to work effectively from home as well as  will have good communication skills and experience of multi‑working, and should preferably have experience working with older adults with functional and organic illness.

The successful candidate will manage a caseload under the supervision of Band 6/7 members of staff.

• Registered Mental Health Nurse
• Car driver / access to a car
• Good communication skills and experience of multi‑working
• Experience working with older adults with functional and organic illness (preferred)

An attractive relocation allowance of up to £8,000 is available to successful candidates.
