Job DescriptionWe are looking for an experienced Care Coordinator to develop and deliver a customised Community Care/Support Service throughout the Buckinghamshire region. The Care Coordinator will organise and arrange individualised care and resources for clients to continue receiving care in their homes and the community. The role demands composure under pressure and the ability to meet deadlines.
The chosen applicant will work at our Amersham office Monday through Friday from 9 a.m. to 5 p.m., with weekend availability required for on‑call coverage. A deep knowledge of the Bucks region is required.
Primary Responsibilities & Duties- Ensure that all applicable policies, procedures, and regulations are followed when providing care to our clients.
- Take responsibility for the carer roster allocations to guarantee that every client’s demands are satisfied.
- Assess prospective clients to determine their unique care requirements.
- Examine current care plans and reevaluate current clients.
- Communicate with all necessary family, medical, professional, and other resources.
- Deliver top‑notch customer service to all of the company’s internal and external clients.
- Conduct prompt spot checks, supervisions, and care worker evaluations.
- Provide on‑call coverage for after‑hours services as a member of a rotating team.
- Ensure that management and employees communicate clearly and effectively.
- Maintain service timeliness and continuity of care.
- Determine reporting channels and offer solution‑focused feedback.
- Use processes and systems efficiently.
- Advocate for inclusion, diversity, and equality.
- Manage the hours and rotas of employees, ensuring proper breaks, rest periods, travel time, minimum and maximum hours, and accurate electronic timesheets.
- Adaptively fulfil the home care service’s operating requirements and provide community care when needed.
- Transfer employees from one service user to another, if necessary.
