Een zorgorganisatie in Amsterdam zoekt een Trajectbegeleider voor het project Actief in de Wijk. In een klein team ondersteun je klanten met sociale en psychische kwetsbaarheid. Je werkt op flexibele locaties en je stelt de regie van de klant centraal. Vereist is een afgeronde HBO-opleiding binnen de zorgsector. Dit is een parttime functie van 8 uur per week.
