- $17hr + $1200wk Stipend (Tax Free)
- 40hrs Guaranteed Weekly
- AM and PM
- 12 hour and 8 hour Shifts
- 8-week assignment
- Snap Care is seeking a dedicated and skilled Licensed Practical Nurse (LPN)/Licensed Vocational Nurse (LVN) specializing in post-acute care to join our healthcare team at our facility in Auburn , New York. As an LPN/LVN, you will play a crucial role in providing exceptional patient care and contributing to the overall well-being of our patients in a post-acute care setting.
- Valid LPN/LVN license in New York.
- Minimum one year of recent post-acute care experience.
- Sound knowledge of nursing principles related to post-acute care.
- Excellent communication and interpersonal abilities.
- Provide compassionate nursing care to patients in a post-acute care facility, ensuring their physical and emotional well-being.
- Conduct comprehensive health assessments, document medical history, and collaborate on individualized care plans.
- Administer medications, perform treatments, and monitor vital signs for patient safety and comfort. Coordinate patient care by scheduling tests, consultations, and referrals.
- Collaborate closely with registered nurses, physicians, and other healthcare team members.
- Maintain accurate medical records and adhere to safety guidelines.
