Role SummaryThe post holder will work as part of the Axminster Hospice at Home Specialist Nursing Team, to deliver the Axminster Integrated Model for Palliative and End of Life Care for All. This service is commissioned and funded by The Axminster League of Friends, but hosted and managed by the Royal Devon University Healthcare NHS Foundation Trust, as part of the Community Nursing service for Axminster.
This team provides a specialist holistic and patient-centred palliative-care service for all patients who are registered with an Axminster GP and diagnosed as palliative, regardless of disease or complexity, guided by the individual needs and wishes of the patient and their family/carer.
This role encompasses the assessment and advisory skills of a Palliative Care Clinical Nurse Specialist, with practical Registered Nurse “Hospice at Home” skills.
This team integrates seamlessly with the existing community nursing and social care providers, and local primary and secondary care services.

Band 6 Integrated Palliative Care Nurse Specialist (Axminster Hospice at Home)
Are you a dedicated, experienced nurse who always strives to give the highest standard of care to your patients? Do you have a particular interest in palliative care?
We are seeking an experienced and motivated nurse to work as part of the Integrated Community Nursing team to provide enhanced palliative care to the patients in Axminster. This is an exciting opportunity to be part of a professional, specialist palliative care team, who will work alongside members of the wider multidisciplinary team of health and social care colleagues, both in the community and the acute care sectors.
The Axminster End of Life service is based on the Sidmouth and Seaton Integrated Model for Palliative and End of Life Care, working in partnership with Hospiscare. This model integrates the available services within the community into a “one-team” approach to provide the best possible care for all patients who are diagnosed by their GP as palliative, regardless of diagnosis or complexity, guided by the individual needs and wishes of the patient and their loved ones.
This service will provide continuity of care throughout the patient’s journey, enabling people to die in their preferred place, which is often their home.
The Axminster Hospice at Home Service will be managed by the Royal Devon University Healthcare NHS Foundation Trust and funded by the Axminster League of Friends. The service will provide specialist palliative care 7 days a week, 365 days a year, with shifts covering 08.00 - 20.00. By providing specialist end of life care and carer support within the Axminster Community, unnecessary and unwanted hospital admissions will be avoided allowing individuals to die in their preferred place of care.
You will need to have experience in caring for palliative care patients and preferably have Community nursing experience. You will have opportunity to learn and gain experience through clinical reflective practice and continued professional development.
The successful applicants will have good I.T. skills, be comfortable using digital technology, be of a flexible and adaptable personality, and willing to embrace change.
The post-holder must hold a current driving licence and have access to appropriate transport, as the post involves predominantly working independently, maximising place-based care.
This post is a 30 hours/week part-time Integrated Palliative Care Nurse Specialist position, to include weekend and Bank Holiday working on a rota system.
