A leading recruitment agency is seeking an experienced Adult’s Social Worker to join the Learning Disability / Transitions Team in Aylesbury. This role involves conducting Care Act Assessments and supporting young individuals and their families during the transition to Adult Social Care. Ideal candidates will have a strong background in adult social work and excellent communication skills. The position offers a pay rate of £37 per hour and is a contract role.
