- Craft personalised care plans in collaboration with residents, families and healthcare professionals.
- Ensure accurate documentation of care and observations.
- Lead shift team, providing guidance and ensuring clarity of roles.
- Facilitate seamless handovers and maintain continuity of care.
- Perform routine observations and manage specific health needs.
- Administer medications safely and in accordance with MAR sheets.
- Build trusting relationships with residents, families and staff.
- Promote independence and support daily activities.
- Respond to emergencies and participate in reflective practice.
- Encourage engagement and enrich residents’ social life.
- Maintain a safe environment for all.
- Advocate balanced nutrition and hydration.
- Registered General or Mental Health Nurse (RGN or RMN) with an active NMC PIN.
- Experience in a care home is an advantage but not essential.
- Demonstrated ability to connect with residents, families and colleagues in a compassionate manner.
- Excellent communication skills, with clarity, empathy and respect.
