Position: Community Sister / District Nurse

We are currently seeking a Community Sister to join our Teesdale Community Nursing Team. The successful applicant will work across the Teesdale locality and the surrounding areas from an office base. The team covers 8am until 9pm, 7 days a week, on a duty roster. The post holder will be working in the unplanned team, providing a high level of care and assessment to patients requiring UCR.

The role involves providing holistic nursing care to adult patients in their own home or residential care setting. Successful applicants will be required to hold a first level nursing registration with the NMC; additional training specific to the role can be provided.

Main duties of the job

To support coordination and delivery of high-quality nursing care that, where possible, meets identified patient needs, supports the aims and objectives of the integrated health and social care team, and remains within the boundaries of available resources. Predominantly complex clinical care and urgent care response, but also supporting the wider team with planned care when required due to clinical demand. Provides supervision and line management to Band 5 staff nurses, Band 4 nursing associates and Band 3 HCA’s.

The successful candidate will provide a variety of nursing care to patients, including wound care, administration of medication, palliative care, management of bowel and catheter care. The role involves assessment of patient care needs, care planning, delivery of care and review. The successful candidate will be required to work independently as part of the wider nursing team, reporting back to the Team Lead.

To support patient safety and clinical effectiveness, providing expert evidence-based clinical advice and support staff within the team.

  • Communicates complex sensitive, confidential information concerning patients/clients displaying empathy, persuasion and reassurance (Some may have special needs; patients/clients may be hostile, antagonistic).
  • Demonstrates skills for assessing and interpreting complex needs of patients/clients
  • Plans provision of care for patients/clients
  • Assesses, plans, implements and evaluates clinical care of patients/clients
  • Provides specialist advice to patients /clients /carers
  • Develops and implements specialist programmes of care
  • Imparts unwelcome news, care of terminally ill / safeguarding issues
  • Follows policies, makes comments on proposals for change
  • Implements policies and proposes changes to working practices or procedures in own work area
  • Supervises work of others; clinical supervision of staff, students; provides training to others
  • Works within codes of practice and professional guidelines
  • Leads an area or team; manages a caseload in the community
  • Occasional participation in R&D activity
  • Physical Skills
  • Driving, carries out immunisations / restraint of patients/clients using approved techniques; dexterity and accuracy required for, e.g. intravenous injections, syringe pumps and infusion, insertion of catheters and removal of sutures.

Statutory Registration
  • 1st Level Registration with the NMC (Adult)

Qualifications
  • 1st Level Registration with the NMC (Adult)
  • Professional knowledge of community nursing acquired through degree or equivalent level of demonstrable professional knowledge/experience
  • Supplemented by diploma level specialist training, experience, short courses which indicate a clear passion for nursing in the community
  • Physical skills course or equivalent
  • Non-Medical Prescribing
  • Management and leadership course
