An exciting opportunity has now arisen within the Community Specialist Palliative Care Team for a Clinical Nurse Specialist in Palliative Care. In this role, you will work across the Barnsley neighbourhoods, providing supportive palliative care to enhance the quality of life for patients and carers with any life‑limiting illness, including complex physical, psychological and emotional needs.

Job Summary

As Band 7 Macmillan CNS Palliative Care, you will:

  • Manage a caseload of people with specialist palliative care needs
  • Apply expert knowledge of pain and symptom management and complex emotional and psychological care
  • Provide highly specialised advice and guidance to members of the wider health and social care team
  • Design and deliver palliative care education programmes
  • Work effectively as a team member

Main Duties
  • Assess, develop and implement specialist palliative care for patients and families, monitoring and evaluating the effectiveness of care to enable patients to remain in their chosen environment
  • Work closely with neighbourhood teams, primary care and adult social care partners to ensure seamless support for individuals with complex needs
  • Act in accordance with the NMC Code and professionalism standards
  • Take a key role in training, education and staff development, participating in regular supervision and appraisal, and contributing to clinical leadership
  • Provide high level communication and interpersonal skills
  • Work weekends and bank holidays as required

Key Result Areas

1.1 Care/service/function Provision

  • Manage own specific geographical caseload and day‑to‑day management of colleagues in absence
  • Provide highly specialised advice, information, guidance and support to patients, families and the wider care team
  • Lead clinical care in the community, working independently with significant discretion
  • Autonomously assess, plan, implement and evaluate care for patients and families
  • If appropriate, become an independent non‑medical prescriber following registration and CPD
  • Develop supportive relationships with patients and families, using advanced communication skills
  • Monitor and identify complex bereavement issues and refer to appropriate services
  • Act as a resource for information for patients and staff

1.2 Working As Part Of a Team

  • Represent palliative care at primary care multi‑disciplinary team meetings
  • Core member of palliative care multi‑disciplinary team and neighbourhood team
  • Initiate, design and deliver education programmes for multi‑professional groups
  • Mentor newly appointed clinical nurse specialists
  • Contribute to business and strategic planning, network development and regional contacts
  • Participate in audit systems and national research projects
  • Maintain accurate records for continuity of care and data collection

1.3 Managing Self

  • Critically evaluate and disseminate research to promote evidence‑based practice
  • Participate in professional development and clinical supervision programmes
  • Act in accordance with Trust standards, health and safety, and the NMC Code
  • Develop flexible self‑management skills and concentrate in unpredictable work patterns
