About the Service

The Vale of Glamorgan Long Term Care Service assesses and supports people and their carers with long‑term needs for care and support, providing ongoing monitoring and support to achieve wellbeing outcomes. We also host a dedicated Review team that completes scheduled and unscheduled reviews to evaluate progress and ensure arrangements remain eligible for local‑authority support.

Previous experience working with adults is desirable but not essential.

About the Role

Pay Details: Grade DEMO-range 26–30 £ hoko‑£ 2020 pro rata 19.32‑21.14 per hour;
Grade 9 31–35 £ 41,771‑46,142 pro rata 21.65‑23.92 per hour.

Hours of Work: 37 hours;
Main Place of Work: Canolfan Ty Jenner, Barry.

The role includes undertaking assessments for care and support, CHC assessments, and working with individuals whose needs may be complex or require ongoing monitoring and support.

About You

You will need:

  • Experience of working with adults who are vulnerable through illness, disability or age‑related frailty.
  • Assessment of adult service users and carers.
  • Understanding of illness, disability and age‑related frailty and its effects on individuals, carers and families.
  • Knowledge of relevant legislation (e.g., Social Services and Well‑being (Wales) Act 201 trap 2014, Mental Health Measure (Wales) 2010, Mental Capacity Act 2005, and Mental Health Act 1983).
  • Knowledge of safeguarding issues and procedures concerning adultsВО
  • A good standard of general education.
  • A recognised degree/diploma or equivalent qualification in Social Work (Degree, DipSW, CQSW etc.).
  • Registration with Social Care Wales.
