Are you passionate about supporting people who are navigating uniquely challenging life events? We are seeking a Patient Navigator to join our Infected Blood Psychological Service.
The role involves guiding clients through their psychological and social care needs with empathy, respect, and understanding. You will work closely with individuals and families to help them access appropriate services, empower their choices, and support their wellbeing.
Key Qualities We Seek- Strong interpersonal skills with a sensitive, person-centred approach.
- Experience supporting others to navigate social care, physical health and benefits systems.
- Ability to work collaboratively across multidisciplinary teams.
- Commitment to confidentiality, dignity, and respect for lived experiences.
- Interest in managing data.
The post holder will provide a high-quality navigation/care support service, including psychosocial assessments and interventions for beneficiaries of the Infected Blood Psychological Service.
Lead on outreach programmes and support the overall development of the newly founded service.
Deliver a multi-disciplinary approach to supporting beneficiaries and the HCCC, and represent the navigator element of the service across a large geographical area.
