Job posted recently for a Registered Nurse (Band
5) based in Beccles, Suffolk in our Intermediate Care Centre (ICC) providing rehabilitation and Specialist Palliative Care services to patients in the Great Yarmouth and Waveney area. The ICC has 6 consultant led Specialist Palliative Care beds for intensive symptom control and management to maintain quality of life. The post holder will work as part of a multidisciplinary team in the intermediate care service and will be an autonomous practitioner with a high level of clinical reasoning.
The postholder will work 11.5 hour shifts, including weekends and bank holidays. Previous applicants should not apply.

Main responsibilities- Work with the multi professional team and the Nurse in Charge to contribute to ongoing assessment, provide and monitor care in line with service pathways and as agreed with supervising clinicians.
- Monitor the condition and health needs of people in care in partnership with the multi‑professional team.
- Provide holistic and person‑centred care across the lifespan and in various settings; act as a patient advocate with respect for confidentiality and diverse backgrounds.
- Manage patient admission involving clinical experts as relevant; proactive discharge management and assist in decision‑making for complex cases.
- Act as a resource for other health and social care professionals on nursing matters; participate actively in multidisciplinary team meetings.
- Take charge of the service/site in the absence of senior nursing leadership, maintaining a safe environment.
- Model professional behaviour, ensure safe patient care is central to all activities, and provide information to patients and clinicians to aid decision‑making.
- Participate in clinical supervision and maintain a learning environment to maximise education and development in the clinical area.
- Contribute to clinical governance through clinical risk assessment and clinical audits; support effective teamwork and delivery of high‑quality clinical services.
- Contribute to care pathways development, care planning, self‑care promotion and health coaching for patients.
- Support policy/protocol/guideline reviews and alignment with CQC standards; ensure health and safety compliance and appropriate use of clinical equipment.
- Working as a Registered Nurse in a hospital setting
- Experience in community hospital or secondary care settings
- Experience with case management approaches to care
- Excellent written and verbal communication, organisation and time management
- Ability to communicate with diverse groups and work autonomously
- Willing to work flexible hours and locations, including internal rotation (days, nights, weekends and bank holidays)
- Alignment with ECCH values and ability to work across sites as required
- NMC‑registered nursing qualification (Certificate/Diploma/Degree)
- Evidence of continued professional development and a professional portfolio
- Mentorship/teaching and assessing qualification (ENB 998) or equivalent
- Health related degree or further study interest
- Commitment to individualized goal‑directed care and the ability to challenge poor practice
- Knowledge of clinical governance, CQC standards and record keeping (NMC guidance)
- Ability to assess, plan, implement and evaluate nursing needs; prioritisation and delegation
- Clinical judgement for unplanned situations; understanding legal and ethical issues
- Phlebotomy, cannulation and IV therapy/blood transfusion skills
- Ability to complete Systm One training for record keeping requirements
- Experience relating to end‑of‑life care or specialist palliative care nursing
