Telemedicine Nurse Practitioner

Role: Provide primary care using a telehealth platform, evaluating new and returning patients for primary care establishment, acute concerns, and follow-up.

• Deliver patient-centered primary care via telehealth.
• Assess and manage acute needs and coordinate appropriate follow-up.
• Collaborate with primary care team members to support continuity of care.
• Monitor centralized results and use findings to improve patient access and outcomes.
• Coordinate care across the broader healthcare system to ensure seamless transitions.
