In US healthcare, a physician is required to write a note documenting the details of each patient visit. Notes have a standard format and are written in a typical professional style (you can find an example below). The clinical note has multiple purposes, including communication with colleagues and billing for medical services.
Draft a clinical note for the visit in the space below. Please observe the following guidelines:
Include as much detail as possible, whenever the detail is clinically relevant.
Use a professional writing style and your best English grammar.
Feel free to use search or reference material for example to look up drug names.
Do not take any external help like Gen Al (ChatGPT) to draft your note.
