Patient-facing Role:
Engaging directly with patients to provide medication counseling and address any concerns.

Care Home Medication Reviews:
Conducting comprehensive reviews of medications for residents in care homes to optimize their treatment plans.

Complex Polypharmacy Management:
Managing patients with complex medication regimens to minimize adverse effects and drug interactions.

Elderly and Frail Patients Care:
Providing specialized care and support to elderly and frail patients to enhance their well-being.

Quality Improvement Prescribing Indicators (QOF):
Contributing to quality improvement initiatives through adherence to prescribing indicators.

Specialist Expertise of Medicines:
Utilizing specialist knowledge to advise on appropriate medication therapies.

Lead on Person-centred Medicines Optimisation:
Taking a proactive approach to tailor medication regimens to individual patient needs.

Lead on Local Anti-Microbial Stewardship (AMR) within the PCN:
Spearheading efforts to promote responsible antimicrobial use within the Primary Care Network.

QOF Contribution and Enhanced Services:
Maximizing QOF contribution and participating in enhanced services to enhance patient care.

Collaboration with Healthcare Teams:
Working closely with pharmacy teams across primary care, community, and hospital settings to deliver holistic patient care and improve outcomes.
