• Facilitate multi-disciplinary assessments and reviews for children and young people, ensuring adherence to standard operating procedures.
• Conduct comprehensive continuing care reviews and assessments of holistic nursing care needs across different care settings.
• Maintain and develop constructive relationships with a wide range of stakeholders including social care and education services.
• Participate in multi-agency transitional meetings for reviewing care assessments and plans, ensuring a smooth transition across care levels.
• Collaborate on joint commissioning for SEND, focusing on impactful outcomes and incorporating client and family aspirations to develop a holistic understanding of needs.
• Work collaboratively with professionals to assess complex needs and provide consistent care management.
